I go from a base double slot( run and shoot formation)

Coach, I know this is gonna' sound stupid, but we have run this play since the early 80's out of trips.
Just send one of your WB's in motion into trips, or just line-up in trips. Have everyone crack-down, QB reverse pivots while the FB takes off toward trips.
We just call it Trips right 38 Toss, Trips Left 39 Toss.
We actually put a faster runner in the backfield and put our fullback in the slot to crack.
It usually works fine.
Like I said, I know this is really simple and may not be what you're looking for.
