Several of my athletes are going to borrow school equipment (Helmets and Pads) to participate in position specific camps. In an effort to protect liable parties, I am looking for an example of a liability waiver form for equipment to be used by athletes. Please let me know if you have any ideas or examplse as to how this is set up.
