If so how do you block it and does mid-line have to be run to the TE side?

We run midline out of split backs to either side, and have had great success with it. Against the 52, you can run it to the open side. We have the play side guard track to the stong side ILB, and the play side tackle crash down on the near ILB. In fact, you can play games with the DT and DE on the open side by switching between IV and Midline, always picking up the near ILB with the OT.
